The nearest hospital to Mountain Air Force Base is St. Luke92s Regional Medical Center which offers inpatient services and emergency room care. The Medical Center is located about 40 miles from Boise, Idaho. For on-site medical care, the 366th Medical Group Hospital is a 10-bed facility which has specialty areas in internal medicine and pediatrics. Daily appointments are offered at the medical clinic by calling 208-832-1560.



366TH MEDICAL GROUP

The 366th Medical Group is located just south of the main gate, on the northeast corner of the base, east of the base service station. The base is located 10 miles west of Mountain Home, approximately 55 miles south of Boise.

Directions
From I-84 take exit 95 onto Hwy 51 West, through Mountain Home, onto Airbase Highway and stay west on the four-lane main road. The main gate is 12.5 miles west of exit 95.

Mission
The 366th Medical Group provides medical services to support the combat capability of the 366th Fighter Wing at deployed locations as well as the Mountain Home AFB community. The group provides services to the community through a collaborative effort of its five assigned squadrons:

- Aerospace Medical Squadron
- Dental Squadron
- Medical Operations Squadron
- Medical Support Squadron
- Surgical Operations Squadron

The group also maintains a 10-bed hospital and care is provided in the following specialty areas: family practice, internal medicine, pediatrics, general surgery, obstetrics/gynecology, flight medicine, psychiatry, radiology, pharmacy, clinical laboratory, dietetics, optometry, physical therapy and 24-hour urgent care center. Specialty services beyond the capability of the 366th MDG are referred to other military facilities through the aeromedical evacuation system or through local civilian sources. The beneficiary counselor assistance coordinator will assist all members with their TRICARE needs. For more details on health benefits call 828-7803.

Clinics making their own appointments may be called directly. They are mental health, 828-7580 and dental, 828-7300. The medical group strives to provide an acute appointment to every beneficiary that requests one within 24 hours. These acute appointments should be used for active-duty patients with an acute illness; the hospital does not have a classic "sick call."

366TH DENTAL SQUADRON
The dental squadron is a full service dental clinic providing examinations, cleanings, general dentistry, prosthodontics, periodontics, endodontics, treatment of oralfacial pain and oral surgery to our active duty beneficiaries.

The clinic's primary mission is to provide the best healthcare; strengthening worldwide readiness by prevention and treatment of oral disease.

The squadron consists of eight general dentists, 14 active duty technicians, three contract civilians and one civilian.

You can reach the Dental Clinic:
1. During regular duty hours by calling 828-7300.
2. After duty hours a dentist is on call and can be reached through the urgent care center.

Clinic hours are as follows:
Monday and Thursday: 7:30 a.m. to 4:30 p.m.
Tuesday and Friday: 8:30 a.m. to 4:30 p.m.
Wednesday: 7:30 to 11:30 a.m. and 12:30 to 2:30 p.m.

We are unable to provide routine care for family members or retirees; dental insurance is provided by Concordia Dental and enrollment is available through www.ucci.com or picking up an enrollment form at our office.

Family members or retirees who need x-rays taken can walk-in during duty hours with their x-ray prescription.

FAMILY MEDICINE CLINIC
We are the Primary Care Manager, or PCM, for all active duty members stationed at Mountain Home Air Force Base that are not on flying status or air traffic controllers. We are also the PCM for dependents of active duty members stationed here, unless assigned to a PCM off base. We are also the PCM for Tri-Care Prime retirees and their families empanelled with us.

Our professional staff consists of active duty and contract civilian physicians and physician assistants and/or nurse practitioners.

Our support staff consists of active duty medical and administrative technicians, civil service registered nurses and licensed practical nurses , contract civilian RNs, LPNs and medical technicians. Most of our civilian staff are themselves retirees, prior military or dependent spouse and have extensive experience with the unique military environment.

We take pride in the healthcare we provide to those who are serving or have served their country and their dependents.

How to make an appointment you may choose either option:

2. Register and book on-line at www.tricareonline.com (each person needing an appointment must be registered).

How do I get medications refilled/renewed?
There are several options available. Please allow a few days minimum to fill renew medications, so please plan ahead.
1. Medication Refill Line: 828-6947 or 800-446-8980 (if you have refills left on prescriptions filled at this pharmacy).
2. Medication Renewal Line: (208)828-7485, some medications can be renewed by the Pharmacist.
3. Leave a telephone message for your PCM team through the central appointment line 828-7900.
4. Mail order pharmacy: 1-866-363-8667 or www.express-scripts.com/TRICARE . For certain circumstances, this option may be the best, especially if you live far from a net pharmacy.

Referrals/Consults

To make sure your health care records and test results are returned to your PCM, please follow these steps:
STEP 1: When you receive your referral authorization letter, call the provider and schedule your appointment. Please allow 10 business days to receive the letter; however, it is usually available online at www.triwest.com within a few business days.
STEP 2: After making the appointment, call TriWest at 1-866-876-2383. You will be prompted to provide the following information:

-You may select another network provider of the same "specialty" if you wish to see a provider other than the one named in your authorization letter. You can view the network providers on the directory at www.triwest.com
-Track the status of your authorizations and referrals online by registering at www.triwest.com, you can view and print out your consult within a few business days after entered by your PCM.

What if you have cancelled or changed your appointment?
Call TriWest at 1-866-876-2383 to update the referral.

We have walk-in clinic (8:30 to 10:30 a.m. and 1:30 to 2:30 p.m. Monday through Friday) for blood pressure checks, throat cultures, wart removals - after initial evaluation by PCM, administration of ordered shots, shaving waivers - after evaluated by a provider (if the current waiver is older than one year, an appointment is required to obtain a valid waiver), wound checks and suture removal - after evaluated by a doctor.

You may always leave a message for your PCM via the nurse telephone consult. Call 828-7900 and follow the options. If in doubt, talk with the booking clerk and the professional staff at the Tri-West will assist you.

We provide specialty care to ensure the success and safety of specific operators and missions for aviators, air traffic controllers, weapons systems operators and members working in other special operational duty positions.

We provide operational health care that includes casualty care and management, routine health care and clinical services for families under any circumstance required of the mission.

OPTOMETRY
The Optometry Clinic provides primary eye care services for active-duty personnel, dependants, and Tricare Prime beneficiaries. We conduct routine eye examinations for glasses and gas-mask inserts, as well as renewing contact lens prescriptions. We also diagnose, treat, and manage diseases and disorders of the eye.

HEALTH AND WELLNESS CENTER
Our Mission is to promote continual readiness by creating opportunities for individuals to acquire the knowledge and skills to practice healthy lifestyles, increase stamina, and improve overall well being.

HAWC Classes - Call 828-4628 for more information or to sign up

Healthy living program -- 9 to 11 a.m. or 1 to 3 p.m. Tuesdays. Discusses exercise and nutrition strategies to improve fitness and identify motivational factors needed to start and maintain a fit lifestyle. Mandatory for military members with a fitness score less than 75.

Body composition program -- 9 to 10:30 a.m. or 1 to 2:30 p.m. Thursdays. A two-class series with follow ups to help people shed unwanted weight and maximize long-term fitness with a health-wise, sensible approach. Mandatory for military members with a fitness score less than 70 and an abdominal circumference or more than 35 inches for women or more than 40 inches for men.

"Eating to Lower Cholesterol and Hypertension" -- 9 to 11 a.m. Tuesdays or Wednesdays. This three-part nutrition class helps people lower cholesterol and high blood pressure levels and focuses on fat content, portion control, food labels, recipe modification and eating out.

Tobacco cessation class -- 3:30 to 4:40 p.m. Feb. 12, March 19 and April 9. Designed to help people kick the tobacco habit for good. The first class is mandatory for students to receive free nicotine replacement therapy prescriptions.

Healthy back class -- 10 to 11 a.m. the third Wednesday each month. Covers basic back anatomy and gives helpful tips on reducing back strain.

Aquatic physical therapy -- 1 to 2 p.m. Tuesdays and Thursdays. Open only to active duty members listed as physical therapy patients, the class provides another avenue to treat acute and chronic injuries and helps people maintain joint range of motion and fitness
